Community Services

 

The mission of the Community Service team is to empower all participants through case management, support intervention and utilization of community resources.  Emphasis is placed on practices which promote service coordination, collaboration and comprehensive case management.

 

Emergency Housing

The Emergency Housing Program provides assistance to households facing eminent danger of homelessness.  Program applicants are screened for eligibility.  Advisory information, community resource, referral and/or financial assistance are provided to those with verifiable emergencies.  A source of income is required to maintain housing, prior to providing assistance.

 

Continuum of Care

The Continuum of Care program provides intensive case management to voluntary participants who are currently homeless.  The goal of the Continuum of Care program is to assist homeless participants with issues involving housing, employment, income stability and independent living skills.  An individual service plan is developed with each participant, targeting goals which promote self-sufficiency and personal fulfillment. 

 

Participants sign a voluntary six-month contract which may be extended dependent on circumstances.  Case management is a working relationship between participant and case manager.  Benefit to participant is dependent on active involvement of all parties.
