This Mortgage and Water Utility Assistance Program is for residents of Manitowoc, Sheboygan, Door and Kewanee counties. Water utility assistance is also available for RENTERS through this same application (fill out only the parts that apply).

If your income has been affected by the COVID-19 pandemic and you are having trouble making your mortgage loan payment and/or water utility bill, please complete the above application. If contacted that you may be eligible for assistance, start locating/requesting the documents below:

  1. Documentation showing payments made between July 1, 2019 and the date you lost your income due to COVID-19.
  2. Bank statements for all checking and savings accounts for the 3 months prior to application.
  3. All income information, including unemployment, for the 3 months prior to application.
  4. Proof that you were not behind on mortgage payments or water utility bills until a COVID related incident happened.
  5. Photo ID of applicant (head of household only)
  6. Income statements for the 3 months prior to your loss of income due to COVID-19.
  7. Amount of government assistance received due to COVID-19 (other mortgage assistance, unemployment, water utility assistance, other).
  8. Plus, any other documentation requested by Lakeshore CAP.

ELIGIBILITY

  1. The house must be located in Manitowoc, Door, Sheboygan or Kewaunee county.
  2. The applicant must reside in the home for which the assistance is provided
  3. Household income must meet eligibility requirements. Chart located here:

  4. Head of Household must be able to document that unpaid mortgage and/or water utility payments were the result of a COVID-19-related loss.
  5. Household cannot be more than 5 months behind in payments.

Lakeshore Community Action Program (CAP) is proud to announce a Property Assistance Program for residents of Manitowoc, Sheboygan, Door and Kewanee counties. We may provide up to $1000 of Property Tax assistance and help with water bills for eligible households.

If your income has been affected by the COVID-19 pandemic and you are having trouble making your property tax payment, please complete the pre-screen application. Should you be contacted that you may be eligible for assistance, please have on hand the documents below:

  4. Must be able to document that unpaid property taxes were the result of a COVID-19-related loss.
  5. Household must be behind in payments.

Homelessness Gaps & Needs Surveys

The Wisconsin Balance of State Continuum of Care (WIBOSCOC) is a 501(c)3 non-profit organization serving all the counties in Wisconsin except Dane, Milwaukee, and Racine.  ​Our membership is comprised of agencies who serve, support and care about those experiencing homelessness. 

As a WIBOSCOC member, Lakeshore CAP needs your feedback!  We are looking for 25 surveys from area providers and surveys from at least 25 clients. The deadline is November 15th.

The gaps and needs group’s purpose is to research and identify the gaps in services and the needs of the Balance of State’s serving populations and agencies. This group will be able to compile data and provide supportive evidence for identifying these gaps and needs. This group will also be a vital advocate and voice for the populations and service providers within the Balance of State.
